Medical Power of Attorney

The Management and Staff of Nutritional Services invite you to come in and dine with us! We offer a wide variety of options for your dining pleasure.
Case Management is a team of caring professionals, both registered nurses and social workers, who monitor and assist in meeting the needs of our patients from admission to discharge. Our Case Management team carefully and systematically assesses each patient’s plan of care and acts as a liaison to ensure efficient and timely delivery of services, and additionally communicates with insurance providers.
Planning a patient’s discharge may involve arranging for home health care, rental of medical equipment for home use. skilled nursing facility placement including rehabilitation, and alternative living arrangements, such as nursing home care.
Additional services include crisis intervention, adoption services and identification of financial and other resources.
Monday through Friday – 8am – 5pm

Notary Services at Wadley Regional are specifically for the convenience of patients and their families at Wadley Regional Medical Center. Please note we only notarizes hospital related documents such as a medical power of attorney. We do not notarize powers of attorney to handle business affairs, car titles, property transfer or wills.

Social Workers are part of the Case Management Department. They provide services for the psychosocial needs of patients and their families. The Social Work staff is composed of specially trained clinicians that have a Master’s degree in Social Work and are experienced in medical Social Work.
Social Workers Provide:
- Crisis intervention
- Evaluation and referral of patients who are victims of abuse or neglect
- Assistance with long term care planning
- Referrals for concrete needs within the community
- Referrals to appropriate agencies/facilities for substance abuse
- Discharge planning

Pastoral Care & Chapel
Hospitalization is often a time of stress for patients and their loved ones. Wadley Regional’s mission… To enhance the quality of life in the communities we serve is very evident in the services provided by our volunteer Pastoral Care team. Members of our pastoral team represent all religious denominations and are on call 24-hours a day to provide comfort and counseling. Arrangements can also be made for patients who wish to contact their own minister, rabbi or priest.
The Chapel at Wadley Regional Medical Center is located on the main floor between the main lobby and the emergency department.
The Chapel offers a quiet and peaceful place for solitude, meditation and prayer. Prayer request forms are available on the chapel altar and Wadley’s pastoral care members attend to these requests daily. One of the most beautiful works of stained glass can be seen in Wadley’s Chapel, handcrafted by a local artist especially for this area. The Wadley Chapel was recently dedicated to the loving memory of Mrs. Carroll (Nancy) Wheeler, who departed this life and years of service to Wadley Regional Medical Center as a member of our volunteer pastoral care team.
The Chapel is open to visitors 7 days a week, 24 hours a day.
