Offering comprehensive and specialized care to help maintain the maximum level of independence, health and well being, the Wadley Senior Clinic is designed to provide an integrated, team approach to develop an individualized care plan for each patient.
The Wadley Senior Clinic is staffed with specially trained healthcare professionals who are uniquely sensitive to the needs of patients 55 years of age or older. The healthcare team is anchored by Geriatricians and Geriatric Nurse Practitioners who work closely with the team of nurses, clinical pharmacists, social workers and dietitians as well as the families to meet each patient’s individual health care needs.

Because our clinic is devoted exclusively to serving older adults, our staff is uniquely sensitive to their needs and abilities. Being able to distinguish between the normal changes caused by aging and changes caused by disease, is important in order to develop the best treatment plan especially when multiple chronic conditions are involved.
The Wadley Senior Clinic is staffed with specially trained healthcare professionals who can assess, coordinate, and monitor all aspects of your healthcare. Each brings unique knowledge and skills to the team and has focused training in geriatrics. The healthcare team is anchored by Geriatricians and Geriatric Nurse Practitioners who will work closely with the team of nurses, clinical pharmacists, social workers and dietitians to meet each patient’s individual health care needs. Above all, you will be treated with kindness and compassion by our devoted team.
In addition to the medical providers and nursing staff, the clinic will have a social worker and a dietitian. In an effort to assist patients with remaining as independent as possible, the Social Worker will help to address needs of daily living and provide appropriate referrals. The Social Worker will work directly with patients, their families and physicians to assess individual patient needs and to provide assistance with such needs as medications, homemaker / personal care, transportation, support groups, financial aid, and much more. Our dietitian will work individually with patients to assess nutritional needs and develop a personal diet plan to help achieve specific goals. So whether the goal is simply healthy eating or disease prevention, the diet plan will be designed specifically for you.

The first office visit to the Wadley Senior Clinic will be a time for our healthcare team to conduct a complete history and physical as well as a nutritional, mental and psycho-social assessment. This will enable them to evaluate your health care needs and develop a plan of care. Also a review of your medications and a memory/mental status assessment will be conducted. As part of your care, the medical team will provide ongoing guidance and recommendations to help you stay as healthy as possible and make needed referrals to specialists when the need arises.
